The example practice books 200 visits a week across the three physicians at an average of R$350 a visit (the middle of its own price list), and we use 4.33 weeks to a month throughout.

No-shows: at the global average rate of 23%, 46 scheduled visits a week don't happen. At R$350 that's R$16,100 a week, about R$69,700 a month of scheduled revenue that doesn't arrive.

Unanswered calls: say the desk takes 300 calls a week and one in five rings out during check-in or after hours. That's 60 calls; if one in four of those was a booking, that's 15 visits, R$5,250 a week, about R$22,700 a month.

The doctor on the phone: five interruptions a day at six minutes each is two and a half hours a week of physician time. At R$400 an hour that's R$1,000 a week, about R$4,300 a month.

Recall: 250 patients a year are due an annual visit that nobody calls about. If 40% would book when asked, that's 100 visits a year, R$35,000, about R$2,900 a month.

Together, about R$99,600 a month is leaking. We don't claim all of it. Reminders move attendance by roughly 14% relative in the evidence, so with the waitlist backfill and a same-day rebook we take a third of the no-shows (R$23,200), half the unanswered bookings (R$11,350), all of the recall (R$2,900) and the doctor's phone time (R$4,300): about R$41,750 a month recoverable, or roughly R$500,000 a year, for the example practice. Yours is built from your own logs in the first two weeks.

Where we could be wrong
  • The 23% is a global average across specialties and countries; a well-run private practice may already sit well under it, and the recovered number shrinks with it.
  • Average visit value varies enormously between a family consult and a specialist procedure. We'd use the practice's own billing split, not one blended figure.
  • Reminder effects in the literature are for text messages. Our assumption that WhatsApp performs at least as well is reasonable in Brazil, where it's the default channel, but it is an assumption.
  • Call volume and the share that would have booked are assumptions. Two weeks of the practice's call log replaces both.
